Nephrotic Syndrome in Adults

06.01.2011

waktu kat opis, rasa lemah sgt badan..nak duduk tak kena.baring tak kena..bercakap x der mood..badan rasa betul2 weak..so decide mintak balik ajer lar..so mintak kat boss balik awal and trus ajak en suamiku pg klinik untuk buat checkup..sbbnya rasa makin lemah dan terlalu lemah..lagi satu rasa pelik sgt..bila terkencing..air kencing keluar macam air buih sabun..berbuih2 sgt..macam lain dr kebiasaan nyer..

balik dr opis dalam kul 2..trus lar g klinik bersama en suami..so kat klinik tu terangkan lar kat doctor tu..dia pon suruh lar check air kencing..and dapatlar resultnya..kencing ader terlalu banyak protien.++++ protien yang ader..menunjukkan ader masalah lar..

so doctor advice trus pergi hospital..decide ngn en suami untuk g Ampang Puteri Specialist Hospital..

Keluar jer dr klinik td..hati dah rasa sayu sgt..takut sakit lama datang smula.sbb tanda2 pertama dah ader..tapi cool lagi..paling takut kena tahan ward..yer lar skrg dah ader anak..kena pikir anak lagi..pastu dah lar penakut..ddk ward sensorang..mesti mlm2 menangis punya lar..

Sampai jer hospital trus pergi kat amergency..sbb da ader surat dr klinik td..so process nya mudah sket..jumpa doctor dan terangkan smula masalahnya..awal2 skali doktor dah advice kena tahan ward..muka dah rasa nk nangis sgt2..takut lar kena tahan ward..maybe serik kot sbb kecik2 mmg dah slalu sgt ddk hosp..lg pon sapa nk jaga tecikmama tu..mesti rindukan dia..so try dulu discuss ngn doctor tu..aper2 pon perkara wajib adalah check darah dulu..

tunggu dekat sejam..keputusan darah pon sampai..mmg sah..darah pon ader problem..uhuhuh..hati dah makin takut..so lepas doctor kat amergency tu refer ngn doctor buah pinggang..decide suh dtg esoknya pulak..so appointment kul 8 pagi..

07.1.2011

pagi2 lagi dah pergi hospital...jumpa dengan pakar buah pinggang kat sana. Dato Dr Zaki Morad bin Mohamed Zaher..doctornya sgt ramah dan penerangan yang diberikan sangat puas hati..daripada hati gundah gulana jadi sejuk sket lar bila dengar penerangan terperici dia..mula2 mmg dia suh msk ward tp disebabkan x der bengkak2..so leh buat kat umah jer..macam2 alasan anak kecik lar..itu lar..ini lar..ahahha..kena buat urine 24 hours..so semua air kencing untu hari ahad tu kena simpan dlm tong yg dia bagi..x leh buang..and baru lar diaorg tau keputusan sebenar..and buat ujian darah sekali lagi..ini tapisan ujian darah kali ketiga..

10.1.2011

Tarikh yang agak menyedihkan bagi saya..sedihnya..Ini lar tarikhnya..saya disahkan semula oleh doctor menghidap Nephrotic Syndrome...masalah buah pinggang..
Sebenarnya waktu kecik dulu..zaman2 sekolah..mmg saya dah pernah hidap sakit ni..tapi alhamdulillah 10 tahun lepas doktor telah sahkan sembuh..menurut doktor, jarang sekali penyakit ini akan kembali semula bila dah dewasa..tapi kita manusia hanya merancang..tuhan yang menetukan..Aku terima takdirmu ya Allah..

Sebelum ni memang saya seboleh-bolehnya mengelak makan makanan yang boleh menjadi penyebab sakit ni..makanan tajam2, air2 manis, makanan masin2 semuanya saya jauhkan..tapi satu jer tak boleh sgt nak elak..ahahha..apa lagi fast food lar..dengan mcd, kfc, nandos, kenny rogers..aduh,..yani..yani..

tapi menurut doktor, pantang paling utama adalah stress..jgn stress.. tapi aku ni kan cengengeng..manja sgt..sikit2 nak amik hati..pastu nangis..sah2 lar stress..apa nak buat..dah mmg sifat tu dr kecik kan..

apa2 pun alhamdulillah..walaupun disahkan menghidap sakit tu, tapi kali ni tak lar teruk macam kecik2 dulu..kalau dulu, kaki ni dah membengkak dengan besar sekali..muka dan tangan pon bengkak..mata yang bulat akan jadi sekecil2nya..merening2..sakitnya hanya Allah yang tahu..duduk dalam wad tak pernah pon sekejap..mesti lebih 2 minggu..harap2 kali ni tak lar seteruk itu..tapi kali ni badan hanya letih..lesu..dan mudah pitam..tu yang bahaya..kalo nak kena..x kira tempat dan waktu..

waktu nak tunggu dapat keputusan tu..hanya Allah jer yang tau..yer lar penyakit2 serius ni mmg sesuatu yang paling susah untuk kita terima kan..tapi dengan sokongan en suamiku yang sentiasa memujuk dan menenagkan..alhamdulillah..akhirnya dapat gak terima..walaupon waktu doktor dah buat pengesahan tu mmg rasa nak nangis sgt..kena buat2 happy and x der apa2 gak supaya dia pon kuat dan tak risau..

menurut doktor, buah pinggang saya ni bocor..so kalo saya makan makanan yang mengandungi protien, dia x masuk kat darah..dia terus kuar melalui air kencing..so kandungan protien dalam darah tak cukup..tu yang boleh sbbkan letih..lesu..tekanan darah tinggi..kalo protien dah x cukup kat darah kandungan kolesterol pon meningkat lah..itu lar nephrotic syndrom.kat bawah ni ader serba sedikit pengetahuan tentang penyakit ni..untuk panduan bersama..

What is nephrotic syndrome?

Nephrotic syndrome is a condition marked by

  • very high levels of protein in the urine, a condition called proteinuria
  • low levels of protein in the blood
  • swelling, especially around the eyes, feet, and hands
  • high cholesterol

What causes nephrotic syndrome?

Nephrotic syndrome results from damage to the kidneys' glomeruli—tiny blood vessels that filter wastes and excess water from the blood and send them to the bladder as urine.

When the glomeruli are working properly, they keep protein in the blood from leaking into the urine. Healthy kidneys allow less than 1 gram of protein to escape through the urine in a day. In nephrotic syndrome, the damaged glomeruli allow 3 grams or more of protein to leak into the urine during a 24-hour period.

As a result of this protein loss, the blood is deficient. Normal amounts of blood protein are needed to help regulate fluid throughout the body. Protein acts like a sponge to soak up fluid into the bloodstream. When blood is low in protein, fluid accumulates in the body’s tissues rather than circulating. The fluid causes swelling and puffiness.

Nephrotic syndrome can occur with many diseases. In adults, the most common causes are diabetic nephropathy and membranous nephropathy. In older adults, the most common cause is amyloidosis. Prevention of nephrotic syndrome relies on controlling these diseases. Frequently, however, the cause of nephrotic syndrome is unknown.

How is nephrotic syndrome diagnosed?

Your doctor will need blood and urine samples to evaluate your condition.

A high level of protein in a spot urine sample may indicate nephrotic syndrome. The doctor may order a 24-hour collection of urine in order to get a more precise measurement.

Blood tests may show low levels of protein. If kidney damage is advanced, waste products such as creatinine and urea nitrogen may build up in the blood.

Once nephrotic syndrome is established, the doctor may recommend a kidney biopsy—a procedure in which tiny pieces of the kidney are removed for examination with a microscope. The biopsy may reveal the underlying disease so that the doctor can determine a course of treatment. If a person has had diabetes for some time, and the patient history and laboratory tests are consistent with diabetic nephropathy, a biopsy is rarely necessary.

How is nephrotic syndrome treated?

In addition to addressing the underlying cause, treatment of nephrotic syndrome focuses on reducing high cholesterol, blood pressure, and protein in urine through diet, medications, or both. Two groups of blood pressure medications—angiotensin-converting enzyme (ACE) inhibitors and angiotensin receptor blockers (ARBs)—also protect the kidneys by reducing proteinuria.

Some people may benefit from limiting protein in their diet to reduce the buildup of wastes in the blood.

Nephrotic syndrome may go away once the underlying cause, if known, has been treated. In children, 80 percent of cases of nephrotic syndrome are caused by a condition called minimal change disease, which can be successfully treated with prednisone. However, in adults, most of the time the underlying cause is a kidney disease such as membranous nephropathy or focal segmental glomerulonephritis, diseases that are treated with corticosteroids, immunosuppressive drugs, and, in some cases, cytotoxic agents. Unfortunately, these treatments do not always bring about remission of nephrotic syndrome. Depending on the disease, as many as half of the patients may develop chronic kidney disease that progresses to end-stage renal disease. In these cases, the kidneys gradually lose their ability to filter wastes and excess water from the blood. If kidney failure occurs, the person will need dialysis or a kidney transplant.
